Container ship MSC TERESA reported fire in engine room in the morning Jun 16 north of Ushant, English Channel, while en route from Sines Portugal to Le Havre France. Fire was taken under control and extinguished by crew. French Navy helicopter conducted two surveillance flights over container ship, last one at around 1500 UTC found no smoke and normal temperature. The ship resumed sailing towards Le Havre. As of 0500 UTC Jun 17, the ship was in process of berthing at Le Havre.

Reefer cargo ship HOLMFOSS ran aground in Lerstad area, east of Alesund, Norway, in the afternoon Jun 17. In several hours, the ship was refloated with tug assistance, and taken to Ellingsoufjord for inspection. Understood no damages, or serious damages, were found, the ship resumed voyage to Alesund. As of 1500 UTC Jun 17, HOLMFOSS was approaching Alesund.

Container ship AS ROSALIA collided with fishing boat under Yavuz Sultan Selim Bridge, Istanbul, Bosphorus, in the afternoon Jun 17, while transiting the Strait in northern direction, en route from Istanbul to Constanta Romania. Fishing boat sank, of 33 fishermen on board 1 died, 2 were injured. The ship completed the transit and was anchored at outer Istanbul anchorage, Black sea.

Early in the morning Jun 17 MRCC Mumbai was alerted by phone call from general cargo ship MANGALAM, reporting that the ship was pushed aground by storm at coast of Raigad district south of Mumbai. Second officer said the ship is taking on water, 16 crew are about to abandon the ship. Indian Coast Guard sent a CG ship and scrambled helicopter, crew were strongly advised to stay on board and wait for rescue. In challenging weather conditions, CG ship’s lifeboat or boats, and helicopter, managed to rescue all crew. MANGALAM was en route from Mumbai to Rebdanda port, understood she was disabled due to mechanical failure, and couldn’t stand against onshore wind and waves.

Container ship DEVON reportedly, suffered crack in portside fuel tank area, followed by oil leak, on Jun 16, some 260 nm SE of Chennai, India, in Bay of Bengal. The ship duly informed MRCC Colombo on accident, she was drifting for some time, resumed sailing, then stopped for the second time, and as of 1730 UTC Jun 17, was under way again. She’s en route from Colombo Sri Lanka to Haldia, India, with 382 containers on board. All fuel in damaged fuel tank was pumped to another tank. Allegedly, some 10 tons leaked.

As part of its latest Safety Flashes, IMCA analyzes an case of failure of EGS valve stem on dive helmet.

The case:

A member reported an incident in which the emergency gas system (EGS) valve handle on a commercial diving helmet broke off. At the time of the failure the dive helmet was undergoing pre-dive inspection on deck.

What went wrong:

A dive technician stripped the valve and discovered the break to be located at the valve stem – where the threaded portion meets the square section. Both parts of the valve stem, adjacent to the break, were noted to be pitted and a fine hairline crack was noted.

The failed component was replaced with a new spare. Following a gas leak and function test, the dive helmet was placed back into service. The failed component was sent ashore for further investigation.

Findings:

The member arranged an independent laboratory review of the material to try and identify the root cause. The finalised laboratory report indicated that failure occurred as a result of pitting and stress corrosion cracking under the action of the chlorides present in salt water.

Lessons learned:

  • Following its investigation our member took the following actions:
    • The company preventative maintenance system (PMS) was modified to require NDE dye penetrant testing of EGS valve stems for all base and field diving helmets at a minimum of 12 monthly intervals.
    • Internal instructions were issued stating that if the post-dive, inspection, or maintenance routines recommended by the manufacturer highlighted any concerns, the relevant valve stem(s) should undergo immediate NDE dye penetrant testing.  If any anomalies are then detected, the component should be discarded and replaced.

This week, the first COVID-19 vaccines for seafarers were administered as part of the Dutch vaccination program for crew.

Namely, on 16th June, Cora van Nieuwenhuizen, Minister of Infrastructure and Water Management visited the vaccination campaign where more than 20 seafarers received their J&J vaccine.

Because seafarers are a hard-to-reach target group who are confronted worldwide with limited freedom of movement due to COVID-19 measures, customization is necessary to be able to vaccinate them (in time).

….Royal Association of Dutch Shipowners (KVNR) announced.

To remind, the Dutch government set up the Dutch vaccination program for seafarers and has invited the Royal Association of Dutch Shipowners to take charge of this programme.

The application procedure for vaccinations opened in the afternoon of 9 June, while the first vaccinations started on 15th June.

Fire erupted in engine room of ro-pax ferry GREIFSWALD on Jun 18 in Black sea, the ship being en route from Chornomork Ukraine to Batumi Georgia. Not much details available, understood the ship was disabled, adrift for several hours, later taken on tow and towed to Chornomorsk anchorage. As of 1300 UTC Jun 19, the hip was approaching Chornomorsk, escorted or towed by two tugs. No injures reported.
